4. Scroll down the block containing the STATEMENT OF AVERAGE GROSS COMPENSATION PAID BY HERBALIFE TO US MEMBERS IN 2013. YOU MUST SCROLL THROUGH THE ENTIRE TEXT BELOW BEFORE ACCEPTING IT.
5. Then on YOUR SPONSOR INFORMATION section you need and it is very important to place the following information with the codes and instructions we provide to you:
-Country Of Residence *: (Check their country of origin)
-Sponsor's Herbalife ID Number *: IMPORTANT place the following code: (PR00000223)
-First 3 Letters of Sponsor's Last Name *: (VAZ)
-Have You Already Purchased an Herbalife Member Pack (HMP) from your Sponsor? *: (Answer NO)
